Department of Health Levies Fines for Violations

WATERTOWN, N.Y.  —  The state Department of Health listed the following food service violations and fines in documents first obtained by Newzjunky.com.

Bonnie Castle Resort, Outer Holland St., Alexandria Bay, paid a total of $2,200 in fines.

· $200 - There were rodent droppings noted in the main kitchen in the floor by the old "appetizer station" equipment, on a tray of spices on a dry storage shelf and on a shelf in the dry storage room next to the popcorn storage. There was evidence rodents had chewed into a 12.5 pound bag of popcorn next to the droppings.

· $200 - In room #226, the sink was leaking and water was pooling on the floor. A mildew odor was noted. Many rooms were noted with water stains on the ceilings, stains on the carpets and caulk in the bathroom in disrepair, missing or dirty. Screens were missing on some rental room windows. Must and mildew odors were noted in rooms 109, 235, and 226.

· $100 - The floors, shelving and walls in the linen room were dirty. Some shelving was not sealed, therefore, not easy to clean. The ceiling over the dryers was not finished, insulation was exposed. There were ants on the floor in the linen room under the garbage. The door to the linen room was not tight fitting. There gaps around the door.

· $100 - In room #245, there was a large stain on the box spring for the bed near the window. The caulk in the bath tub and shower area were discolored with possible mold growth. The access gate on the jacuzzi tub was not attached exposing dirt and filth located inside the access area. There was a large hole next to the toilet where an access panel was missing.

· $100 - In room #337, the box springs for the bed and the carpet were stained. There was a large black spot under the door of the refrigerator possibly due to the presence of mold. There were cob webs around the door to the balcony. Food particles were noted on the floor under the beds. The small sink faucet was linking. Some grout in the bathroom was in disrepair. The cabinet under the sink was dirty with debris and dust.

· $200 - The floor tiles and grout in the main kitchen were in disrepair. In many areas, the tiles were cracked or broken and the grout was missing.

· $100 - At the time of inspection no U.S. Coast Guard approve ring buoys were supplied at the outdoor pool.

· $300 - Operation reports for the outdoor pool were not complete in that chlorine residual readings were recorded only one time per day, not the required minimum of three times per day. The violation was repeated on Aug. 29, 2011. The chlorine residual had not been recorded at all for six days prior to the Aug. 29, 2011, inspection.

· $150 - An orthotolldene test kit was being used for chlorine and pH testing of the pool chemistry instead of the required DPD type test kit. This violation was repeated on Aug. 29, 2011.

·
$50 - All three gate entrances to the pool were not shut or latched at the beginning of inspection. One gate near the chlorine storage room was propped open with a piece of brick. This violation was repeated on Aug. 29, 2011.

· $50 - The depth markings were not visible inside the pool at all locations around the pool as outlined in 601.29 6.1.1. This violation was repeated on Aug. 29, 2011.

· $100 - The warning sign posted at the outdoor pool did not state "Two or more adults, 18 years of age or older, must be present at the pool when the bathing facility is in use, with at least one adult on the deck." This violation was repeated on Aug. 29, 2011.

· $50 - At the time of the inspection, there was a large piece of concrete on the side of the pool (part of the deck) which was broken and loose. Holes for placement of pool equipment in the deck which were open and a possible tripping hazard. This violation was repeated on Aug. 29, 2011.

· $300 - At the time of inspection, the chlorine residual in the outdoor pool was less than 0.6ppm.


Northwood Manor, Route 11, Adams, paid a 500 in fine.

· $500 - On June 23, 2011, the mobile home park was being operated without a valid permit to operate a mobile home park. The last valid permit to operate expired March 8, 2007.
