Vaccinated outside of New York State? You can still get the Excelsior Pass

News release

WATERTOWN, NY —  In order to accommodate individuals that received their
Covid-19 vaccine outside of NYS who would like to obtain and use the Excelsior Pass, the Jefferson County Public Health Service (JCPHS) will be available to enter your vaccination information into the New York State Immunization Information System (NYSIIS) at 531 Meade Street, Watertown on June 25th 9am-12pm. This will be on a walk-in basis only during these times. You must present the CDC Covid-19 Vaccination card along with picture identification and have a NYS address. As JCPHS is a healthcare facility, facial coverings are required regardless of vaccination status. Those that have been vaccinated for Covid-19 in
NYS are already entered into NYSIIS and are able to obtain their Excelsior Pass on their phone or computer.

Excelsior Pass is a fast, free, voluntary and secure way to present digital proof of COVID-19 vaccination or negative test results that is supporting New Yorkers building back better and safer than ever before.
